CLASS ACTION ADMINISTRATION
If you are an heir of a deceased claimant, send copies of the following documentation:
  • Death certificate
    AND
  • Last will and testament
    or
  • Order from probate court establishing beneficiary(ies).

For securities class action claims, a statement showing an account was a joint tenancy with right of survivorship from the month of the decedent’s death may be submitted if there is no order from probate court establishing beneficiary(ies).
